1) Social Engineering - This category refers to the three most common social engineering attacks: email-based phishing, text based smishing, and voice-based vishing.

Social engineering was by far the top human risk identified by respondents as technology alone can only go so far in stopping attacks. While phishing still remains the primary social engineering attack method, we are seeing a rise in both the number and sophistication in smishing and vishing attacks. This increase is in part because as organizations are getting better at detecting and stopping phishing attacks. It is also because fewer organizations have control over or visibility into employees’ mobile devices.

2) Artificial Intelligentce (AI) This is the third year that AI has appeared as a top risk and unsurprisingly so. What is new is that, in the past two years, AI has moved from the fourth top human risk to a strong second. The issue is not that AI is inherently vulnerable or unsafe. Rather, it’s that AI is so new that organizations are struggling to figure out not only how to use it, but what policies and controls should be in place to manage those risks. In addition, employees are moving beyond Generative AI (GenAI) and are now looking into and leveraging vibe coding and agentic AI, often without the security team knowing or people using unauthorized solutions.

3) Incorrect Handling of Sensitive Data More people are handling more data in increasingly complex ways, and with more rules governing how that data must be handled. Add the complexity of different sensitivity levels, the different ways the data can and cannot be shared. Connect it all with cloud and AI-based solutions, and you can see just how complex secure data handling can be.

4) Passwords/Authentication How people authenticate and manage their passwords is a top risk as account takeover has become a primary attack vector. Why hack into a computer when you can simply log in?

Cyber attackers also know that leveraging legitimate credentials to gain access creates far less noise and makes unauthorized activity much more difficult for security teams to detect (a term often called living off the land). Interestingly, for the past two years we have seen this risk drop as a priority. Two years ago this was rated as the second top risk, last year this was rated as the third top risk.
